I'd have thought that getting it to detonate would be fairly straightforward... possibly radio control or swallowing a chemical to set it off.

If they using something that small surely its simpler and more secure just to put it in their pocket. Besides, you're going to need pretty potent stuff to deal with the fact the explosion is going to be a bit nullified by the fact its inside someone.

Best application as a means of assassination with the comic cartoon effect.

Also remember, our stomachs are much smaller than that of a drug mule. You're never going to be able to swallow as much as you can wear--the only advantage might be getting it into someplace you couldn't walk in wearing a vest of explosives.
